Prepare for Your First Visit
We know that getting ready to visit THOCPA for the first time can be stressful because there’s so much to do. Here’s a list that will make it easy to prepare for your first appointment.
Initial visit and Your Patient Portal
Make check-in faster by completing all the forms and questionnaires before your visit. If you have not been provided a login, please contact the center and request your secure login credentials. You can click here for the THOC Patient Portal.
Organize medical records
Your patient access specialist will review all of the necessary medical documents we would like you to bring for your first appointment.
Please bring the following information to your first appointment:
- One valid photo identification (driver’s license, state ID, military ID or passport)
- Current insurance card(s)
- A list of your current physicians with addresses and phone numbers
- A detailed list of ALL your medications, including:
- Why you take them
- Dose amount
- How often you take them.
- Bring your pharmacy’s contact information.
- A list of your allergies
- Medical Power of Attorney and Living Will, if completed. Learn more about Advance Care Planning and the important process of talking about your goals and wishes in terms of your health care.
- Any additional information your Patient Access Specialist has asked you to bring

Plan your travel and lodging
We can arrange domestic and international travel to and from Dallas and help you find a place to stay close to the THOCPA.
- Contact Patient Travel Services:
Your first visit may take a day or so while tests are completed and a care plan developed. Your length of stay will depend on your treatment plan, test scheduling, and you will learn more after you arrive. Visit our contact page to learn more about traveling to THOCPA.
Patients and families may need to stay in Dallas for five to seven days. If you are coming from out of town, please keep your travel plans flexible.
What to bring with you
In addition to the medical information your patient access specialist has gone over with you, here is a list of other items to consider bringing.
- Driving and parking directions provided
- A small notebook for writing notes and questions
- Comfortable clothing, shoes and something to keep you warm, such as a sweater or jacket
- A laptop or mobile device. You can use our free wireless network to connect to the Internet.
- Snacks and drinks. We also have many places to eat and find refreshments on campus.
- A book or a magazine to read
What to expect when you get here
We know it can be overwhelming. If you have questions once you arrive, ask for Welcome Center information representatives or ask any employee with an ID badge for help. We are here for you.
Your first visits to THOCPA will involve registration and meeting with your care team and doctor.
Remember to bring your valid photo ID and your insurance card. A patient access specialist will help you:
- Validate your parking ticket (if required)
- Sign consent forms
- Review your insurance benefits
- Take a photo for your medical record
Meeting with your doctor and care team
Your doctor and care team will review your medical history and perform a physical exam. Further testing may be scheduled by your care team. Your doctor and care team will discuss next steps with you and your family. Your care team includes:
- Case managers
- Physical and occupational therapists
- Registered dieticians
- Social work counselors
- A patient advocate
